PHONE CONTACT.  Different people react differently to the same treatment, thus it is important to let the doctor know about any difficulties or discomfort you are experiencing with medications.  If you are having an urgent or emergent reaction, please seek immediate medical attention.  If the matter can wait, please leave a message and the doctor will attempt to return the call within 24-48 business hours.   If the discussion is complex, an appointment will need to be made.

If a more lengthy telephone conversation is required (10 minutes or more) or frequent, short phone calls (3 or more in one month), then you will be charged for the time at the same rate as appointments, with a minimum of $25.

REFILLS.  In most cases, refills will not be provided to pharmacies by phone without an appointment and refills will not be allowed in excess of 3-4 months at a time.  Please contact the clinic to make an appointment, prior to or, when you pick up your last available refill.

CANCELLATION POLICY.  In order to provide better service to our clients and due to the size of our clinic, we do not overbook to compensate for no shows therefore we must charge for missed appointments. Please give 48 hours (2 working days) notice of cancellation so the doctor may reschedule the appointment time. For less than 24 hours notice, the full cost of the visit will be charged – this also applies to future missed appointments without notice.

URGENT NEEDS.  Due to the size of this clinic, we currently will not have crisis, pager or after-hours services available.  If you foresee needing this, or more availability of services, an appropriate referral can be made to other providers/clinics in the community.

If unforeseen urgent matters occur, you can leave a message on the clinic voicemail and expect a return call, in most cases, within 24-48 business hours.  If your emergency is so acute that waiting for a response is not appropriate, please call 911 or go to the nearest Emergency Department.  UNI Crisis Service can also be reached 24/7 at (801) 583-2500.

EMAIL & PRIVACY.  Client information will not be shared, sold or otherwise disclosed to any other party except as requested by the client in writing. The law protects the relationship between a client and a physician with the exception of the following situations:

  • Suspected child abuse or dependent adult or elder abuse, for which we are required by law to report this to the appropriate authorities immediately.
  • If a client is threatening serious bodily harm to another person/s, we must notify the police and inform the intended victim.
  • If a client intends to harm himself or herself, we will make every effort to enlist their cooperation in insuring their safety. If they do not cooperate, we will take further measures without their permission that are provided to us by law in order to ensure their safety.
  • Therapists are required by law to report incidences of sexual misconduct on the part of other therapists.
  • Additionally, our clinical staff may consult with their supervisors.
  • We abide by HIPAA practice standards.

There are limits to protecting your confidentiality over the internet.  Please consider the information you submit over the internet prior to sending.  Please do not use email or “contact us” form to send messages of a personal or confidential nature. Even though we do our best to keep our email confidential, email is copied, archived, and retransmitted continuously as part of normal processing. Because of this, please use email only to request information about our services, or other non-confidential matters.

There are several ways you can communicate with us without having to send information over the Internet: in person, by phone, fax or mail.

Email is intended in this clinic for administrative purposes i.e. changing appointment times, contacting therapist or office to request an appointment, or confirm appointment times. Please recognize that email is not completely secure or confidential. If you choose to communicate via email, be aware that emails are retained in the logs of internet service providers. While it is unlikely that someone will be looking at these logs, they are in theory available to be read by the system administrator(s) of the internet service provider. Please be aware that any emails received or sent can become part of the legal record.
